Looking for a case manager with home care experience to provide oversight of the care/services furnished by employees to Interim HealthCare patients and clients.
Collaborates with the Director of Health Care Services (DHCS) to develop and implement processes to ensure the efficient and effective achievement of patient/client outcomes in accordance with physician orders, applicable law and regulations, accepted standards of clinical practice, payer requirements and Interim HealthCare policies and procedures.
Essential Functions:
Supervises, evaluates, motivates and develops professional and paraprofessional employees. Works primarily with all Pediatric clients in home care.
Minimum Education & Experience Requirements:
· Graduate of an accredited school of nursing and licensed to practice as an RN in the state in which employed. License is active and in good standing.
· Prefer at least 3-5 years of experience as a home care RN working mainly with pediatric patients and at least one (1) year of supervisory experience and 2-3 years hospital or home care experience.
· Ability working with low tech and high tech pediatric patients.
· Follows accepted standards of clinical practice.
· Uses clinical decision making to provide oversight and guidance to the healthcare team to efficiently and effectively manage the assigned patient population.
· Contributes to business growth by presenting information, education and support to external customers as requested.
· Completes other assignments as requested and assigned.
